Welcome to Cats on Call! We look forward to meeting you and your cat(s). Please fill out the following form before your visit so we can provide the best possible care to your feline friend. Thank you and we cannot wait to meet you!

Patient Information

Please let us know which company in this line. Include policy number

For insurance submission, many companies make it easy for you to submit insurance through an app or on your own. We will always make sure to send you your invoices. We will send records to your insurance company as well if required. For Trupanion insurance holders we can submit everything through the clinic

To help us provide the best care we can, please provide a detailed list of the previous veterinary clinics your cat has visited so that we may request records. A full medical history allows us to make the most appropriate recommendations based on your cat's individual needs. This includes proper vaccination recommendations and lab testing. 
Please include the name of the clinic and state (phone number if available), as well as any specialty or ER hospitals your pet has visited.
Please also include adoption records if you have them as many adoption facilities are difficult to obtain records from.

Any chronic illnesses or diseases we need to know about?

Fee Policy

All professional and medical services must be paid in full at the time services are rendered. We accept Cash, Visa, Mastercard, American Express, Discover, Debit Cards, Care Credit, and personal checks. If using a personal check we must have your drivers license number and date of birth on file. As a cat guardian, you will be held liable for the financial responsibility of services that are performed for each cat. Payment is due, in full, at time of service. Unpaid balances will be recovered as deemed appropriate by Cats on Call management.

Cancelling and Rescheduling

If you need to cancel or reschedule your appointment, please notify us with minimum of 24 business hours and be advised that if you miss an appointment, you may be required to leave a deposit equal to the current examination fee when rescheduling. The state of Maine requires all cats over 3 months of age must be rabies vaccinated regardless of indoor/outdoor status. Cats with chronic medical conditions or known history of rabies vaccination reaction will be evaluated on a case by case basis at the veterinarians discretion.
